I have adopted a child who was born in Tennessee. How do I get a new birth certificate for them?

You should submit the following to this office:

  1. 1. A certified copy of the court order of adoption.
  2. 2. A completed Certificate of Adoption.
  3. 3. A check or money order for the required fee. If you want a certified copy of the new certificate please include an additional fee for the desired number of copies.
